We are your medical nucleus

GCM is the quarterback of every patient's care — not just a specialty, not just a referral hub, but the central intelligence that connects, coordinates, and closes every loop. No other practice manages every aspect end-to-end: referral scheduling, specialist follow-up, medication oversight, and result closure. We don't hand the relationship off when something gets complex. We hold it.

IN PRACTICE

A member's specialist orders a procedure. We schedule it, prep the patient, receive the results, integrate them into the record, and call the member with a plan — before they think to ask.

02Pillar Two

We are proactive, not reactive

GCM doesn't wait for patients to get sick. Quarterly chart reviews, scheduled outreach, and anticipatory scheduling shift care from "sick visits" to ongoing health optimization. Your Patient Care Coordinator looks ahead — consolidating upcoming reminders, grouping related appointments, reaching out before patients have to ask. Care comes due, not symptoms after the fact.

IN PRACTICE

Every member's chart is reviewed quarterly by their physician — looking ahead at what's coming due, what trends are emerging in their labs, what conversations need to happen before the next visit.

03Pillar Three

We own the follow-through

A referral is the beginning, not the end. Every order, test, and result is tracked until the loop is fully closed. GCM manages scheduling, prep check-ins, day-of care, results, and next-interval planning — approximately ten steps competitors call "one referral." The patient should never chase Griffin to find out what happened.

IN PRACTICE

A colonoscopy at Tampa General is one phone call from the member — and approximately ten steps from us: scheduling, prep instructions, day-of confirmation, post-procedure follow-up, biopsy results, integration into the chart, and the next-interval reminder.

04Pillar Four

We are always there

On-call physicians ensure 24/7 coverage — not a voicemail, not urgent care down the street; a dedicated, prepared physician ready to respond. Target response is 30 minutes or less. Team coverage creates sustainability — no single physician burns out, and no patient is left without support.

IN PRACTICE

A member calls at 2am from a hotel in Europe with chest pain. Within thirty minutes, a Griffin physician is on the line — with their full chart, their medication list, and the local resources to direct them safely.

05Pillar Five

We know you personally

The designated Patient Care Coordinator relationship is not administrative — it is relational. Your PCC knows your life events, communication preferences, and health journey. Communication adapts to the person: phone calls for sensitive or complex situations, text for routine updates. Team structure ensures continuity — patients interact with a small, consistent circle, not a rotating cast.

IN PRACTICE

You don't introduce yourself when you call. Your PCC already knows you're traveling next week, your spouse just had surgery, and you prefer text for routine but phone for anything that matters.

We grow with you

Griffin's 20+ years of combined physician experience, national network access, and institutional affiliations with Mayo Clinic, Cleveland Clinic, Moffitt, MD Anderson, Hopkins, Dana-Farber, and Memorial Sloan Kettering create a compounding knowledge base. IN PRACTICE

Members who joined Griffin in 2009 are still members today. Their physician may have changed; the standard, the team that owns their care, and the relationship has not.

FoundationalCare

The basics, done extraordinarily. Most concierge practices stop at primary care — we begin there. These four dimensions are the foundation on which everything else rests.

Primary Care

The foundation: your physician, deeply familiar with your history, available when it matters. Visits are unhurried — typically 30 to 60 minutes. The kind of relationship where you don't have to re-introduce yourself, summarize your history, or hope your records get faxed somewhere in time.

Preventive Medicine

Urgent Care

Same-day access to your own physician for acute concerns — not a stranger at a clinic, not a triage line. The doctor on the phone is the doctor who knows you and your history.

Hospital Privileges

OUR PROVEN PROCESS

How membership works.

From your first inquiry through your third year and beyond — the Griffin Way is a process, not a transaction. Here is what happens, step by step.

STEP ONE

The Inquiry

Membership begins with a conversation, not a form. You tell us what brought you here — current physician, complexity of your health, what’s been missing — and we tell you honestly whether Griffin is the right fit. We turn down prospective members whose expectations we cannot meet. Mismatched membership is the costliest kind.

STEP TWO

Meeting Your Physician

You meet your physician in an unhurried introductory visit — typically 90 minutes. The goal isn’t to diagnose. It’s to begin a relationship. By the end, your physician knows your story, your communication preferences, your goals, and your concerns. You leave knowing the same about them.

STEP THREE

The Comprehensive Workup

In the first 60 days, we build a longitudinal baseline. Comprehensive labs, age- and risk-appropriate screening calendar, family history captured in detail, current medications reviewed. Where helpful, we coordinate with specialists in our national network — Mayo, Cleveland Clinic, Moffitt — to refine the picture.

STEP FOUR

Your Designated Patient Care Coordinator

Within the first 30 days, you’re assigned your dedicated Patient Care Coordinator — your operational partner through every appointment, referral, lab, and medication adjustment. Your PCC knows your communication preferences, your family situation, your travel patterns. They are the relationship that makes the relationship work.

STEP FIVE

Ongoing Care, Quarterly Reviews, Lifelong Continuity

Every quarter, your physician reviews your chart — looking ahead at what’s coming due, what’s trending in your numbers, what conversations need to happen. Care is proactive, not reactive. As your life changes — and it will — your care adapts. Members who joined Griffin in 2009 are still members today. Their physician may have changed; the standard has not.
